We would recommend that you fill out the electronic registration forms below and click on the register with NYC Vipers. In the event, you would like to use the standard Vipers form, click to download below then print and fill out by hand.
Register in person at Our Lady Of Lourdes - Mar 8th 2pm - 6pm
Print and bring with you the following forms:
- Medical Release
- Liability
- Code of Conduct
- Copy of Birth Certificate
